contracted individuals (employment purposes) for a minimal fee of $35.00. This Corporate PVC ID
Card is a personalized, double sided, full color card printed on durable 30 mil plastic for the highest
level of wear resistance. This card is considered a secondary form of identification and should be
accompanied by licensing or certification documents as well as a primary form of identification (i.e.:
state driver's license, ID card or government issue passport). Upon ordering an identification card,
please forward a "jpeg" face shot measuring approximately 2 inches, with your full name, and a
contact telephone number for personal information needed.
